Please print it, fill it out, and bring it to your first session.

Also note that the adolescent form should be filled out by the adolescent with the likely assistance of a parent/guardian. An adolescent age 16 or 17 years of age should sign their initial paperwork along with the parent/guardian.

Make sure your margins are set for 1 inch for top/bottom and right/left.
